Use this request form to transfer ownership of Qualtrics surveys.
Audience
All Qualtrics users. This request form requires ONID login.
Request
Click the "Request Service" button; this will start a new request. Once the request is completed you will receive a confirmation with your ticket number and a link to review your request.
