Community Network customers may use this form to request assistance moving computer equipment to a new location.

Please submit this form as soon as you can. After you submit this form, please call the Service Desk at 541-737-8787 to confirm scheduling. For urgent move requests, call the Service Desk at 541-737-8787 and ask to speak to a supervisor.
