Community Network customers may use this form to request assistance moving computer equipment to a new location.
Please submit this form at least 5 days before a move. For more urgent move requests, call the Service Desk at 541-737-8787 and ask to speak to a supervisor.

Request
Click the "Request Service" button; this will start a new request. Once the request is completed you will receive a confirmation with your ticket number and a link to review your request.
