Overview

Request the activation of a voice / phone service.

Audience

OSU departments and external organizations that have contracted with OSU Telecommunication Services.

Request

Click the "Request Service" button on this page to start a new request. A form will appear asking for specific information needed to complete your request. You will receive an e-mail with the details you submitted and the unique request number.
